Confirm that you reached the named area or ending shown beside the badge. Rejoin once and repeat that trigger. If it still does not appear, record the missing badge and current game version before changing your route.

Are there only six checkpoints?
Not necessarily. Six named depth milestones have badges, but intermediate checkpoints may exist without one.
Does reaching Abyss unlock Find My Phone?
No. Find My Phone unlocks with the Phone Ending, not just by reaching Abyss.
